Appointment Cancellation & No Show Policy FormPlease enable JavaScript in your browser to complete this form.Everyone’s time is valuable; so that I may best serve all my clients, please be advised of the following:You may reschedule, cancel, or make changes to an appointment using the online booking system up to 2 days/48 hours before the beginning of your appointment, then the system locks out. If it is less than 48 hours before your appointment, you can still make changes without a penalty, simply contact Melissa directly at 206.715.6303 by calling or texting. This helps to keep the online booking site up-to-date and offers others the opportunity to schedule a last minute appointment.There is no fee for cancelling an appointment with a 24 hour notice. Simply contact Melissa at 206.715.6303 and she will remove it from the booking system upon your request.Cancellations with less than a 24 hour notice may be subject to a fee equal to the service. Exceptions are made for circumstances involving flu-like or COVID related symptoms, a contagious illness, or a family emergency. Women clients, if your monthly cycle begins and you do not want to modify your Brazilian service, your appointment may be rescheduled with no fees imposed, or you may keep your appointment and receive a modified service wearing the disposable panties provided to you. Full Brazilians will NOT be performed if you are on your period. NO Exceptions.No Shows: In the event of a no-show, the client will be responsible for paying the full amount of their scheduled treatment regardless of the service cost. Square will either apply the no-show service charge or the client will be sent an electronic invoice for payment. If the appointment time can be filled, the no-show client will be notified and will not be charged for their missed service.Late Arrivals: If you might be, or will be more than 15 minutes late to your appointment start time, please contact Melissa directly at 206.715.6303. Your appointment may need to be rescheduled depending on the start time for the next appointment. This ensures that each client has adequate time to receive their service. If it is past 15 minutes and the client has not made an attempt to contact Melissa directly, they will be marked as a no-show. This policy ensures we have a mutual agreement; just like you are planning for me to perform the service that you set time aside for, I am counting on you to show up for your scheduled appointment.Please be advised that children are not permitted in the treatment room unless they are receiving a service. On or before a minor’s first appointment, a parent or guardian must be present to complete and sign a Parental Consent Form to release permission for services requested. This form will be kept on file and does not require an adult to be present for future appointments.Clients are not permitted to bring a guest in the treatment room when a service requires the client to disrobe. Exceptions are made for a parent or guardian accompanying a minor and attendants assisting clients with a medical disability.Thank you for your understanding.
